Patients with significant dental anxiety who cannot tolerate treatment despite nitrous oxide, have trauma histories making traditional dentistry impossible, experience panic attacks or severe physical anxiety responses, have avoided necessary care for years due to fear, or require deeper sedation (IV or general anesthesia) for safe treatment delivery.
We provide the full sedation spectrum: Oral Conscious Sedation for moderate anxiety, IV Sedation for high anxiety or longer procedures, and General Anesthesia for severe phobia or extensive treatment.
Surgical cases requiring advanced expertise or sedation: impacted wisdom teeth (especially those close to inferior alveolar nerve), deeply fractured teeth requiring sectioning and bone removal, teeth with curved/dilacerated roots or severe ankylosis, multiple extractions requiring efficient completion, and full-mouth extractions for immediate or conventional denture placement.
Dr. Koniouchine's extensive surgical training and sedation expertise allow safe, efficient extraction of teeth that may be challenging in a general practice setting.
Patients with complex medical conditions requiring anesthesiologist oversight during sedation, advanced monitoring and emergency preparedness, coordination with medical specialists, or modified treatment protocols for safety.
Medical conditions we routinely manage include: cardiovascular disease, poorly controlled diabetes or endocrine disorders, respiratory conditions (severe asthma, COPD, sleep apnea), bleeding disorders or anticoagulation therapy, seizure disorders, history of adverse reactions to anesthesia, organ transplant recipients, and cancer patients.
Our board-certified anesthesiologists (Dr. Hesham Talab, MD MSc PhD FRCPC FASE and Dr. Asad Mirghassemi, MD MSc FRCPC) provide medical-level patient assessment and intraoperative management that ensures safety for patients others might consider "too risky."
Children requiring sedation beyond nitrous oxide: very young patients (ages 2-6) unable to cooperate for treatment, children with severe dental anxiety or negative past experiences, extensive restorative needs best completed in one sedation visit, special needs children requiring adapted approaches, and patients with sensory processing challenges or autism spectrum disorders.
We provide compassionate pediatric care with sedation options ranging from oral sedation to general anesthesia.
Adults and children with developmental, cognitive, or physical disabilities requiring sedation for safe treatment delivery, adapted communication and behavior management, extended appointment times and patience, and caregiver involvement and support.
We welcome patients with autism spectrum disorders, intellectual or developmental disabilities, cerebral palsy or other physical disabilities, dementia or cognitive impairment, and communication disorders.
Patients with extensive treatment needs who benefit from completing multiple procedures under sedation in one or two appointments, comprehensive treatment planning for full-mouth restoration, combination of surgical, restorative, and prosthetic procedures, and implant placement and restoration.

Include: patient demographics (name, date of birth, contact information), brief clinical description of case and treatment needed, medical history summary, reason for referral, recent radiographs (digital preferred), and any previous failed treatment attempts or patient concerns.
Phone Referral: Call our office at (613) 482-0501. Our administrative team can take referral information over the phone.
Within 24-48 hours of receiving the referral, our team contacts the patient to schedule consultation or treatment appointment, verify insurance coverage and provide cost estimates, answer patient questions about sedation options and procedures, and send new patient forms if not previously completed.
Consultation First (Complex Cases): For extensive treatment, medically complex patients, or when sedation level determination is needed.
Direct to Treatment (Straightforward Cases): For clearly defined cases (e.g., wisdom teeth extraction with IV sedation for healthy patient).
During treatment: appropriate sedation level administered (oral, IV, or general anesthesia), Dr. Koniouchine performs planned procedures efficiently, board-certified anesthesiologists manage sedation for IV/GA cases, patient recovers comfortably in our private recovery area.
After treatment, we send comprehensive report including: treatment completed, sedation level utilized and patient response, any complications or unexpected findings, radiographs (post-operative if taken), post-operative instructions given, follow-up plan and recommendations. This report is typically sent within 3-5 business days post-treatment.
For ongoing preventive and routine restorative care, patients return to your practice. We handle only the referred sedation/surgical procedures, maintaining your role as their primary dental home.
