What Is IV Sedation? Provided by Anesthesiologists

IV sedation—also called intravenous conscious sedation or twilight sedation—delivers anti-anxiety medication directly into your bloodstream through a small needle placed in your arm or hand. This direct route creates rapid, predictable effects that we can precisely control and adjust throughout your entire procedure.

Think of IV sedation as the "goldilocks" level between oral sedation and general anesthesia—it's deeper than medication taken by mouth, but lighter than complete unconsciousness. Most patients under IV sedation enter a state sometimes called "twilight sleep." You're technically conscious and can respond to questions, but you're so deeply relaxed that you may drift in and out of actual sleep. You'll have little to no memory of your procedure afterward, and you certainly won't remember any anxiety or discomfort.

What makes IV sedation particularly valuable is its adjustability. Unlike oral sedation where you take a pill and get a fixed level of relaxation, with IV sedation we can increase or decrease your sedation depth instantly throughout your treatment. If you need deeper relaxation, we administer a bit more medication. If we need you more responsive, we let the current dose lighten slightly. This precise control makes IV sedation exceptionally safe and allows us to maintain your ideal comfort level from start to finish.

Perfect for Severe Anxiety and Complex Procedures

At our Ottawa dental clinic, IV sedation serves patients with severe dental phobia, those requiring extensive dental work, or anyone who simply wants the deepest relaxation possible without general anesthesia. It's our most popular option for wisdom teeth removal, multiple dental implants, and full mouth rehabilitation.

When IV Sedation Works Best:

Severe dental anxiety or dental phobia
History of panic attacks at dental visits
Previous traumatic dental experiences creating lasting fear
Complex surgical procedures (impacted wisdom teeth, multiple implants)
Extensive treatment needs (multiple crowns, full mouth restoration)
Very long procedures (2+ hours)
Strong gag reflex preventing comfortable treatment
Patients who want zero memory of their procedure
Multiple procedures combined in one visit
Special needs patients requiring deeper sedation

Complete Sedation Spectrum

IV sedation is part of our comprehensive range of comfort solutions:

Nitrous Oxide (Laughing Gas)

Mild relaxation for lower anxiety.

Oral Sedation

Moderate relaxation with oral medication.

IV Sedation

Deep relaxation or light sleep with continuous adjustment. (Current Page)

General Anesthesia

Complete unconsciousness with zero awareness, administered by board-certified medical anesthesiologists (Dr. Hesham Talab, MD MSc PhD FRCPC FASE and Dr. Asad Mirghassemi, MD MSc FRCPC).

We can also combine IV sedation with nitrous oxide for enhanced comfort. For complex cases, our medical anesthesiologists provide the highest level of sedation safety and expertise.

We'll help you choose the sedation level matching your anxiety, procedure complexity, and personal preferences.

The Science of Intravenous Comfort

IV sedation medications work by enhancing GABA (gamma-aminobutyric acid), your brain's primary calming neurotransmitter. When we administer these medications directly into your bloodstream, they reach your brain within 30-60 seconds, creating rapid onset of relaxation and sedation effects.

Common Medications We Use:

Midazolam (Versed)

Our primary sedative. Creates amnesia (memory loss), reduces anxiety, and produces muscle relaxation. Fast-acting and predictable, with effects wearing off within 1-2 hours.

Fentanyl

A pain medication that works synergistically with midazolam to enhance comfort. Very short-acting—effects last about 30-40 minutes.

Propofol

For deeper sedation when needed. Creates more profound unconsciousness similar to general anesthesia but lighter and more easily controlled.

Why IV Administration Is Superior:

When you take medication orally, it must pass through your stomach, get absorbed through your intestinal walls, and travel through your liver before reaching your bloodstream and brain. This process takes 30-60 minutes and varies tremendously based on what you've eaten, your metabolism, and individual factors.

With IV administration, medication enters your bloodstream instantly and reaches your brain within seconds. This provides:

  • Immediate onset of effects (no waiting period)
  • Precise dosing control (we see your response in real-time)
  • Adjustable sedation depth (can increase or decrease instantly)
  • Bypass of digestive system (more predictable effects)
  • Higher reliability (100% of medication reaches your bloodstream)

Levels of IV Sedation:

We can provide different depths ranging from moderate to deep:

Moderate (Conscious) Sedation: You're very drowsy and relaxed but can respond to verbal commands and physical stimulation. You may sleep but wake easily.

Deep Sedation: You're on the edge of consciousness—sleeping most of the time but can be aroused with significant stimulation. Limited memory and awareness.

We customize your sedation depth based on your anxiety level, procedure requirements, and personal preferences discussed beforehand.

What to Expect During Your Visit

Step 1: Before Your Appointment

Preparation ensures your safety and optimal sedation experience. You'll receive detailed pre-sedation instructions:

Fasting Requirements: No food for 8 hours before your appointment (typically from midnight if you have a morning appointment). Clear liquids allowed up to 2 hours before. This prevents nausea and potential aspiration complications.

Medication Review: We'll review all your current medications. Some may need temporary adjustment or discontinuation. Bring a complete list including supplements and over-the-counter medications.

Transportation Arrangements: You must have a responsible adult drive you to and from your appointment and stay with you at home for the rest of the day. You cannot drive, operate machinery, or make important decisions for 24 hours.

What to Wear: Comfortable, loose clothing with short sleeves or sleeves that roll up easily for IV placement. Avoid jewelry and leave valuables at home.

Step 2: Arrival at Our Office

When you arrive, we'll confirm your medical history and fasting status, answer any remaining questions, and take baseline vital signs. You'll settle into a comfortable dental chair where we'll place monitoring equipment—pulse oximeter on your finger, blood pressure cuff on your arm.

A trained sedation team member will place a small IV line, usually in your arm or hand. This involves a brief pinch similar to having blood drawn. Once the IV is in place, there's no ongoing discomfort—you won't feel medications being administered.

Step 3: During Your Procedure

We begin administering sedation medication through your IV. Within 30-60 seconds, you'll feel a warm, relaxed sensation washing over you. Many patients describe feeling like a heavy, comfortable blanket has been placed over them.

As the sedation deepens, your eyelids may feel heavy and close. You'll enter "twilight"—aware on some level but deeply relaxed and unconcerned. Your anxiety disappears. Time seems to compress dramatically. You may drift in and out of light sleep.

Throughout your procedure, we continuously monitor your heart rate, blood pressure, oxygen levels, and breathing. We can adjust your sedation depth at any moment—deepening it if you seem uncomfortable, lightening it if we need you more responsive.

Your dentist performs your dental work while you rest peacefully. You may hear voices or sounds but they'll seem distant and unimportant. Most patients later describe feeling like they closed their eyes for a moment and then their procedure was finished.

We still use local anesthetic: Even under IV sedation, we numb specific treatment areas to ensure zero discomfort. Most patients don't remember receiving these injections.

Step 4: Recovery

After your dental work is complete, we stop administering sedation medication. You'll gradually become more alert over 30-60 minutes. During this time, you'll rest in our recovery area under continuous monitoring.

You may feel groggy, disoriented, or emotional as you wake—this is completely normal and passes quickly. Some patients feel euphoric; others simply want to continue sleeping. We'll ensure you're stable and comfortable before discharge.

Step 5: Going Home

Your designated driver will take you home. You'll need someone to stay with you for the rest of the day.

Important post-sedation instructions:

  • Rest for the remainder of the day (sleep is normal and encouraged)
  • Have someone stay with you until effects fully wear off (usually 6-8 hours)
  • Do not drive, operate machinery, or make important decisions for 24 hours
  • Avoid alcohol for 24 hours
  • Start with clear liquids, then light foods as tolerated
  • Take any prescribed medications as directed
  • Follow specific post-procedure care instructions for your dental treatment

Most patients feel completely normal by the next morning, with no "hangover" effects. We'll call to check on your recovery within 24 hours.

Comprehensive Safety Protocols

Your safety is our absolute priority during IV sedation. We maintain rigorous protocols that meet or exceed all provincial standards:

Continuous Monitoring: Pulse oximetry, blood pressure, heart rhythm, and breathing monitored throughout your procedure
Emergency Equipment: Full emergency cart with defibrillator, oxygen, airway management equipment, and emergency medications immediately available
ACLS Certified Team: Team maintains current ACLS (Advanced Cardiac Life Support) certification
Reversal Medication: Flumazenil immediately available to reverse benzodiazepine effects if needed
Pre-Sedation Screening: Thorough medical evaluation including review of conditions, medications, allergies, and previous sedation experiences
Board-Certified Anesthesiologists: Dr. Hesham Talab, MD MSc PhD FRCPC FASE and Dr. Asad Mirghassemi, MD MSc FRCPC available for complex cases

Who Should Avoid IV Sedation:

While IV sedation is safe for most patients, we recommend alternative options for:

  • Severe respiratory conditions (uncontrolled asthma, COPD)
  • Severe obstructive sleep apnea
  • Pregnancy
  • Allergy to sedation medications
  • Certain heart conditions
  • Extreme obesity (BMI >45)
  • Some neurological conditions

For these patients, we may recommend general anesthesia with our medical anesthesiologists instead, providing even greater safety margins.

Procedures We Perform Under IV Sedation

IV sedation is ideal for virtually any dental procedure, especially:

Oral Surgery

  • Wisdom teeth removal (especially impacted)
  • Multiple tooth extractions
  • Dental implant placement (single or multiple)
  • Bone grafting
  • Sinus lift surgery
  • Complex surgical extractions

Restorative Dentistry

  • Multiple crowns or bridges
  • Full mouth rehabilitation
  • Extensive cavity treatment
  • Complex root canal therapy

Cosmetic Procedures

  • Multiple veneers
  • Smile makeovers
  • Full arch cosmetic reconstruction

Periodontal Surgery

  • Gum grafting
  • Periodontal surgery
  • Crown lengthening

Pediatric Dentistry

  • Extensive treatment for older children/teenagers
  • Special needs patients

Multiple Procedures, One Visit

One of IV sedation's greatest advantages is enabling comprehensive care in a single appointment. Patients commonly complete procedures that would normally require 4-6 separate visits—having everything done comfortably while sedated. This dramatically reduces:

  • Total time off work or school
  • Number of numbing injections received
  • Overall stress and anxiety
  • Recovery periods (one instead of multiple)
  • Total cost (one sedation fee instead of multiple)

Valuable Investment in Your Comfort

IV Sedation: $500 - $900

Per appointment, depending on duration and complexity

IV sedation typically costs starting at $500-900 per appointment, depending on the duration and complexity of your procedure. While this represents a higher investment than other sedation methods, the benefits often justify the cost for appropriate patients:

  • Deep, reliable sedation for severe anxiety
  • Ability to complete extensive work in one visit
  • Little to no memory of treatment
  • Precise adjustable control throughout
  • Higher success rates for complex procedures

Most dental insurance plans cover IV sedation at 50-80% when medically necessary—particularly for:

  • Documented severe dental anxiety or phobia
  • Complex surgical procedures (impacted wisdom teeth, multiple implants)
  • Special needs patients
  • Procedures that would be unsafe or impossible without sedation

We provide comprehensive documentation supporting insurance claims. We offer direct billing to major insurers and accept the Canadian Dental Care Plan (CDCP).
