Crown lengthening procedure

What Is Crown Lengthening?

Crown lengthening is a surgical procedure that exposes more of your tooth by removing excess gum tissue and sometimes reshaping the underlying bone. Despite its name, this procedure doesn't actually make your tooth longer—rather, it reveals more of the tooth that was previously hidden beneath gum tissue.

The procedure serves both cosmetic and functional purposes. Some people seek crown lengthening for aesthetic reasons—to create a more balanced smile when excessive gum tissue makes teeth appear short. Others need it for restorative reasons—to expose enough healthy tooth structure to properly support a crown, bridge, or filling. Sometimes both goals overlap.

Making Crown Lengthening Stress-Free

At our Ottawa dental clinic, we believe no one should avoid needed care due to fear. That's why sedation dentistry is fundamental to how we approach crown lengthening.

Nitrous Oxide (Laughing Gas)

Mild relaxation for patients with minimal anxiety. Effects wear off within minutes. Learn more about nitrous oxide

Oral Sedation

Medication creates moderate drowsiness and anxiety reduction. You'll have limited memory of the procedure. Explore oral sedation

IV Sedation

Deeper sedation keeps you in a twilight state—profoundly relaxed with minimal awareness. Our most popular option for crown lengthening. Most patients remember nothing. Discover IV sedation

General Anesthesia

Complete sleep with zero awareness, administered by board-certified medical anesthesiologists Dr. Hesham Talab, MD MSc PhD FRCPC FASE and Dr. Asad Mirghassemi, MD MSc FRCPC. Ideal for severe anxiety or extensive crown lengthening. Learn about general anesthesia

Multiple Teeth in One Visit

With appropriate sedation, we can complete all necessary work in a single surgical session—one procedure, one recovery period.

Cosmetic and Functional Benefits

Cosmetic Crown Lengthening

  • "Gummy smile" correction—when excessive gum tissue covers too much of your teeth
  • Uneven gum line—when gum tissue sits at different heights on neighboring teeth
  • Short or small-looking teeth—if your teeth naturally appear small due to excess gum coverage

Functional Crown Lengthening

  • Insufficient tooth structure for restorations—when a tooth is broken below the gum line or lacks enough exposed structure for a crown to grip
  • Decay below gum line—if cavities extend beneath gum tissue
  • Fractured teeth—when teeth break off at or below the gum line
  • "Biologic width" issues—your teeth need adequate space between where a crown ends and where bone starts

Is This Procedure Right for You?

Good candidates include:

  • Patients unhappy with short teeth or excessive gum display
  • Anyone needing crowns but lacking sufficient tooth structure
  • Patients with teeth broken below the gum line
  • Those with decay extending beneath gum tissue
  • Patients experiencing gum inflammation around crowns due to biologic width violation
  • People with uneven gum lines

Sometimes crown lengthening isn't appropriate. If the tooth root is too short, removing bone might compromise stability. Your comprehensive examination determines the best approach.

What to Expect

On surgery day, we start by administering your chosen sedation. Once comfortable, we thoroughly numb the surgical area. We carefully make small incisions to separate gum tissue from teeth, allowing access to the bone beneath. We remove excess gum tissue as needed, then may remove small amounts of bone to expose more tooth structure. For cosmetic crown lengthening, we sculpt the gums to create an even, attractive contour. Once we've exposed the appropriate amount of tooth structure, we reposition the gum tissue at its new level and place sutures.

Surgery Time:

  • Single tooth: 45-60 minutes
  • Multiple adjacent teeth: 1-2 hours
  • Extensive treatment: 2-3 hours

Healing Process

First 24-48 hours: Some discomfort, swelling, and minor bleeding are normal. Take prescribed pain medication or over-the-counter pain relievers. Apply ice packs to minimize swelling. Sleep with your head elevated. Avoid strenuous activity for 3-4 days. Don't smoke.

Diet: Stick to soft, cool, or lukewarm foods for the first week. Avoid crunchy, hard, spicy, or acidic foods. Chew on the opposite side.

Oral Hygiene: Do NOT brush the surgical area for the first week. Very gently rinse with prescribed mouthwash or warm salt water starting 24 hours after surgery. After one week, resume very gentle brushing.

Healing Timeline:

  • Days 1-3: Maximum discomfort
  • Days 4-7: Significant improvement
  • Days 7-14: Sutures removed
  • Weeks 2-4: Most healing complete
  • Weeks 4-8: Gums mature
  • 6-8 weeks: Ready for final crown (functional cases)
  • 2-3 months: Complete healing and final cosmetic results

Why Crown Lengthening Works

Crown lengthening offers significant benefits:

  • Enhances smile appearance with balanced tooth proportions
  • Enables restorations by providing necessary tooth structure
  • Improves gum health by correcting biologic width violations
  • Saves teeth by exposing broken or decayed structure
  • Provides long-lasting results (permanent once healed)
  • Creates foundation for dental work

Success rates reach 95%.

Comprehensive Smile Transformation

Crown lengthening often works beautifully with other procedures:

  • Veneers or crowns for complete smile makeovers
  • Teeth whitening to brighten your newly visible smile
  • Orthodontics to address both tooth position and gum proportions
  • Gum grafting when some teeth need more coverage while others need less

Investing in Your Smile

Crown Lengthening Costs:

  • Simple cases: Starting at $500-800 per tooth
  • Multiple teeth (cosmetic smile enhancement): Starting at $1,500-3,000
  • Complex cases: Starting at $1,200-2,000 per tooth
  • Sedation fees additional

Insurance coverage varies: Functional crown lengthening (needed for restorations) typically covered at 50-80%. Cosmetic crown lengthening (purely aesthetic) usually not covered.

We provide direct billing to major insurers and accept the Canadian Dental Care Plan (CDCP).
